What it is
The goals of this strategy are to:
Develop a data collection and funding strategy.
Collect data related to other Access to Care strategies, focusing on at-risk populations.
Share results and develop a data continuation plan for the next CHA and CHIP.
What it does
By creating a cohesive approach and system to support to data sharing, the Vibrant Valley Data Cohort can support collaborative efforts that lead to improvements in access to care for the County as a whole.

The Vibrant Valley Data Cohort is responsible for implementing this strategy.
